Referrals can come from any source – from parents, a GP or a hospital – as long as the child’s parent or carer has given their consent.
Application
We welcome the referrals of babies, children and young people who meet our acceptance criteria and who live in the London Boroughs of Barnet, Camden, Enfield, Haringey, Islington or Hertsmere. We will consider referrals from outside our catchment area on a case by case basis.
A referral can come from any professional source, or from the parent or carer themselves.
It’s important that it’s discussed with the family before submission. And because we recognise it can be hard to know the right time to refer, we welcome an informal discussion.
To make a referral, simply download and complete the Referral Form and email it to our Care Team at noahs.referrals@nhs.net. Or, post it to us at Noah’s Ark Children’s Hospice, The Ark, Byng Road, Barnet EN5 4NP.
Assessment
Referrals are assessed by our multidisciplinary panel who consider the child’s diagnosis against our criteria.
We may need to speak to professionals involved in caring for the child and may contact you for additional information before we can decide whether the child meets these criteria.
We will write to the referrer with our decision and families will be contacted within 5 working
days.
