Making a Referral

Noah's Ark follows the National Children's Hospice criteria and any child within the five boroughs who is affected by a life-threatening or life-limiting condition will be eligible for our services.

Families can refer themselves or ask a professional to contact us. The referal form will request details of a doctor who can be contacted if necessary to confirm whether the child's medical condition meets the criteria.
